Job summary

Summit Health Care is seeking a Social Services Director for its Skilled Nursing Facility. The role focuses on identifying residents’ psychosocial needs and coordinating access to services to maximize independence, dignity, and quality of life.

Ideal candidates have experience in a Skilled Nursing or Long-Term Care setting and will lead the Social Services Department, conduct care conferences, and manage discharges to ensure continuous, person-centered care.

Qualifications

• 1 year of social services experience in a Skilled Nursing or Long-Term Care setting preferred.
• Desire to be part of a team that works, grows, and excels together.
• Manage and direct the Social Services Department, and identify and provide for each resident’s social, emotional, and psychological needs.
• Understand the MDS process with ability to interview, input, and complete social service assessments.
• Accurately assess, document, and provide interventions to meet psychosocial needs of all residents; develop care plans based on assessments.
• Provide effective facilitation of resident care conferences: schedule, letters to families, participate in conferences.
• Facilitate all resident discharges (home health, DME, transfer to other facilities)

Skilled Nursing Facility seeking a Social Services Director

OVERVIEW:

The social services director will work with residents in the facility to identify their psychosocial, mental, and emotional needs, along with providing, developing, and/or aiding in accessing services to meet those needs. In addition, the SSD is responsible for fostering a climate that enables residents to maximize their individuality, independence, and dignity. This shall provide residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.

The ideal candidate will possess the following:

• 1 year of social services experience in a Skilled Nursing or Long-Term Care setting preferred.
