Job summary

Rehabilitation and Long Term Care Facility is seeking a Social Services Director to oversee the Social Services Department and ensure residents' psychosocial needs are met in compliance with relevant laws. You will assess, document, and intervene to support emotional wellbeing and collaborate with medical staff.

The role emphasizes care planning, multidisciplinary relationships, and leadership to improve resident outcomes within a long-term care setting.

Qualifications

• Two years of job-related experience, preferably in long-term care.
• Bachelor’s degree in social work or related field preferred.
• Experience with social work, preferably with patients and families at a nursing facility.
• Detail oriented, computer literate and excellent communication skills.
