CareCore at Westmoreland in Chillicothe, OH, is seeking an experienced Social Services Director to support residents and families through admissions, discharge planning, care coordination, and advocacy. You will collaborate with the interdisciplinary team, ensure regulatory compliance, and advocate for resident rights while guiding care plans and family communications to deliver compassionate, person-centered care.

Experience in discharge planning and transition of care is essential.

Qualifications

• Current LSW license strongly preferred.
• Experience in long-term care or skilled nursing preferred.
• Strong communication and organizational skills.

CareCore at Westmoreland is a skilled nursing and rehabilitation facility in the Chillicothe area is seeking an experienced and compassionate Social Services Director to join our team.

This position plays a vital role in supporting residents and families through admissions, discharge planning, care coordination, emotional support, and advocacy throughout their stay.
