Job summary

CareCore at Westmoreland in Chillicothe, OH is seeking an experienced and compassionate Social Services Director to support residents and families through admissions, discharge planning, care coordination, advocacy, and ongoing emotional support.

This role coordinates social services in a long-term care setting, participates in care plan meetings, ensures regulatory compliance, and champions resident rights with an interdisciplinary team.

Qualifications

• Current LSW license strongly preferred.
• Experience in long-term care or skilled nursing.
• Excellent communication and organizational skills.

CareCore at Westmoreland is a skilled nursing and rehabilitation facility in the Chillicothe area is seeking an experienced and compassionate Social Services Director to join our team.

This position plays a vital role in supporting residents and families through admissions, discharge planning, care coordination, emotional support, and advocacy throughout their stay.

Responsibilities:

• Coordinate and oversee social services for residents in a long term care setting
• Assist with admissions, discharge planning, and transition of care
• Support residents and families with emotional, social, and financial needs
