Job summary

CareCore Health in Cincinnati seeks an experienced Social Services Director to oversee social services at the Long Term Care Facility. You will assist residents and families with placement and coordinate discharge planning to ensure smooth transitions.

The ideal candidate has a background in long-term care, strong advocacy, and the ability to lead a small team while partnering with interdisciplinary colleagues for holistic resident care.

Qualifications

• Bachelor’s Degree in Social Work or related field.
• Licensed Social Worker (LSW) in the state of Ohio (preferred).
• Minimum of 2 years experience in a long-term care or healthcare setting.
• Strong knowledge of Medicare/Medicaid regulations and discharge planning.
• Excellent communication, organizational, and documentation skills.

Responsibilities

• Overseeing all social services functions within the facility.
• Assisting residents and families with adjustment to placement.
• Providing support for emotional, social, and psychological well-being.
• Developing and updating individualized care plans.
• Coordinating discharge planning and community resource referrals.
• Ensuring compliance with federal, state, and local regulations.
• Leading and supervising social services staff, if applicable.
• Collaborating with interdisciplinary team members to ensure holistic care.
