Job summary

Manchester Rehab & Healthcare is seeking a Social Services Director to lead psychosocial care for residents and families. The role focuses on assessment, advocacy, discharge planning, crisis intervention, mandated reporting, and care coordination with the Nursing Department.

Ideal candidates will hold a Bachelor's degree in Social Services, Missouri CSW certification, and 2–5 years of acute or long-term care experience, with preference for prior long-term care social work.

Qualifications

• Bachelor's Degree in Social services.
• CSW certification by the Missouri state.
• Knowledge of community resources.
• Experience in Health Care Social work.
• 2-5 years of acute or long-term care experience preferred.
• Experience in long-term care social work also a plus.

Responsibilities

• Provide assessment, advocacy, and discharge planning for psychosocial needs of patients of all ages.
• Demonstrate expertise in crisis intervention, mandated reporting, care coordination of patients with psychosocial risk.
• Coordinate psychosocial needs of residents and their families with the Nursing Department.

We are seeking a full-time long-term care social worker that will provide assessment, advocacy, and discharge planning for the psycho-social needs of the patients of all ages.

This individual will need to demonstrate expertise in crisis intervention, mandated reporting, care coordination of patients with psychosocial risk.

• Must be able to prioritize multiple tasks and possess strong interpersonal skills.
• Must have excellent communication skills and display a professional demeanor with patients, families, interdisciplinary team members, and outside agencies.

Social Services Director

is responsible for coordinating the psychosocial needs of residents and their families. This team works closely with the Nursing Department to meet needs of residents.

Requirements:
