Qualifications

• High school diploma or equivalent and two years of Social Worker experience in a healthcare setting.
• Bachelor's degree in Social Work or related field preferred with five years of related experience.
• Experience in Skilled Nursing Facility is a plus.

Responsibilities

• Assist in preparing residents for discharge and transition.
• Coordinate resident discharges and participate in care conferences.
• Address residents' social and emotional needs during their stay.
• Support a culture of inclusivity and resident-centered care.
