Job summary

Warren Park Health & Living Center in Chicago, IL is seeking a Social Services Designee/PRSC to support skilled nursing residents' social, emotional, and psychological needs. This role focuses on person-centered care and discharge planning in collaboration with families and care teams.

Ideal candidates will have a high school diploma, experience with geriatric populations, knowledge of Medicare/Medicaid, and strong communication skills.

Qualifications

• Reads, writes and communicates in English.
• Strong verbal and written communication skills.

Responsibilities

• Keeps informed about community resources.
• Communicates daily with staff regarding admissions and discharges.
• Discusses choices for advanced directives, financial power of attorney, guardianship, surrogate decision.
• Documents discharge planning and maintains ongoing communication with resident and family.
• Ensures home care needs are met by discharge time, with planning completed 24 hours prior.
• Completes all required assessments and documentation per resident.
• All other duties as assigned.
