Harmony Palos Heights is seeking a Social Services Designee to join our caring team. You will support residents’ social and emotional needs, coordinate discharges, and participate in care conferences to ensure high-quality care.

Applicants should have a high school diploma and at least two years of Social Worker experience, preferably in healthcare. A bachelor’s degree is preferred, but we invest in the right candidate.

Qualifications

• High school diploma or equivalent required.
• Two years of experience in a Social Worker role, preferably in a healthcare setting.
• Bachelor’s degree in Social Work or a similar field preferred.

Responsibilities

• Assist in preparing residents for their future and meet their social and emotional needs.
• Coordinate resident discharges and participate in resident care conferences.
• Ensure residents are comfortable and safe during their stay.

As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Harmony Palos Heights by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
