Harmony Chicago is seeking a Social Services Designee to join our skilled nursing and post-acute care community. You will help prepare residents for discharge, address social and emotional needs, and participate in care conferences to ensure thoughtful resident support.

Minimum qualifications include a high school diploma and 2 years of Social Worker experience in healthcare; a Bachelor's in Social Work is preferred with 5 years of related experience.

Qualifications

• Minimum: high school diploma or equivalent and 2 years in a Social Worker role in healthcare.
• Preferred: bachelor's degree in Social Work or related field and 5 years of related experience.

Responsibilities

• Assist in preparing residents for their future and ensure social and emotional needs are met.
• Coordinate resident discharges and participate in resident care conferences.
• Support residents to ensure comfort and safety during their stay.
