Job summary

Citadel Meadows in Skokie, IL is seeking a Social Services Designee/PRSC to support residents’ social, emotional, and psychological needs, while promoting dignity and growth during skilled nursing care.

The role collaborates with staff on admissions, discharges, and care planning, reviews directives, assists with discharge planning, and ensures compliance with state and federal regulations. Benefits include competitive pay and advancement opportunities.

Qualifications

• High school diploma or equivalent required.
• Experience with geriatric residents preferred.
• Knowledge of Medicare, Medicaid, HMO and private insurance coverage.
• Familiarity with accreditation standards and IDPH/state/federal regulations governing skilled care facilities.

Responsibilities

• Keeps informed about new and existing community resources.
• Communicates daily with staff regarding admissions and discharges to various units and facilities.
• Discusses advanced directives, financial power of attorney, guardianship and surrogate decisions with residents or representatives.
• Documents discharge planning as an ongoing review and coordinates with the resident and family.
• Addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and families.
• Discusses end-of-life care and palliative/hospice options with residents or families.
• Coordinates with Case Manager on insurance and discharge planning.
• Ensures home care arrangements are completed prior to discharge and timely documentation is completed.
