Job summary

Atrium Centers is seeking a Social Services professional to assist the Social Services Director in implementing resident-focused programs and documenting social, emotional, and psychosocial needs. You will coordinate services for residents and families, participate in care planning, and ensure timely documentation for regulatory compliance.

Qualifications

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree).
• Experience working in a skilled nursing or long-term care setting preferred.
• Strong written and verbal communication skills with residents, families, and interdisciplinary teams.
• Ability to assist with documentation, assessments, care planning, and regulatory requirements under supervision of a licensed Social Services Director.
• Compassionate, resident-focused approach with strong interpersonal skills.
• Organized and detail-oriented with ability to manage multiple priorities.
