Job summary

Atrium Centers seeks a compassionate Social Services Coordinator to support resident well-being in a 100% employee-owned organization in West Salem, WI. You’ll work with residents, families, and interdisciplinary teams to address social, emotional, and psychosocial needs with dignity.

You’ll assist with implementation of social service programs, care planning, and documentation to meet regulatory standards. Strong communication and organizational skills are essential.

Qualifications

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)
• Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams
• Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director
• Compassionate, resident-focused approach with strong interpersonal skills
• Organized and detail-oriented, with the ability to manage multiple priorities
• Working knowledge of resident rights, discharge planning, and psychosocial support services preferred
• Positive, can-do attitude with excellent attendance
