Valley View Care Center in Grand Rapids, MI seeks a Social Services Designee to support residents and families dealing with illness and placement. The role involves coordinating social services, referrals, and resident care planning.

The candidate should have a Bachelor's degree in a related field and at least 1 year of social work experience in healthcare; LT care experience is preferred. Strong computer skills and teamwork are essential to provide compassionate, epic care.

Qualifications

• Bachelor’s degree in social/human services or related field.
• 1+ years of social work experience in health care.
• Long-Term care experience preferred.
• Excellent computer skills; experience with electronic charting is a plus.
• Able to multi-task in a fast-paced environment with minimal supervision.
• Knowledge of residents’ rights and able to document interventions clearly.

Responsabilités

• Assist in meeting psychosocial needs of residents and families.
• Coordinate services with other disciplines to support resident well-being.
• Maintain knowledge of residents’ rights.
• Document social service interventions and assessments timely.
• Participate in care planning and multi-disciplinary meetings.
• Inform about changes in roommate or room assignments.
• Assist with admissions, discharges, and MDS testing.

We need your help with scheduling needs:

• Full Time Position
• Monday - Friday position
• No Weekends or Holidays!

How you will help people every day:

• Assist in meeting the psychosocial needs of each resident and their families in coping with the effects of illness, disability, treatment, or stay in the facility.
• Coordinate services with other disciplines within the facility to enable the resident and families to promote their own well-being.
• Maintain a working knowledge of residents’ rights.
• Perform timely documentation of Social Service interventions and assessments in resident charts.
• Participate in resident care management, including resident care plan conferences and developing multi-disciplinary plans of care.
• Inform residents and/or responsible parties of change of roommate(s) and room changes.
• Assist with admissions, discharges, and MDS testing.

What you will need to become a part of our EPIC team:

• Bachelor’s Degree in related human services field, including but not limited to Social Work, Sociology, Special Education, Rehabilitation Counseling, and/or Psychology; LSW preferred but not required
• 1+ years of social work experience in a health care setting working directly with individuals;
• Long-Term care experience is preferred.
• Excellent computer skills; experience with electronic charting a plus.
• Must be able to multi-task and work in a fast-paced environment with little supervision.
• A working knowledge of the basic services provided by skilled nursing and related facilities.
• Excellent customer service skills with a commitment to delivering Excellent, Passionate, Innovative care!
