Valley View Care Center in Grand Rapids, MI seeks a Social Services Designee to support residents and families through social services programming and referrals. The role involves coordinating with the care team, documenting interventions, and assisting with admissions and discharges.

Formación

• Bachelor’s degree in related human services field; LSW preferred but not required.
• Minimum 1 year of social work experience in a health care setting.
• Long-Term care experience is preferred.
• Excellent computer skills; experience with electronic charting is a plus.
• Ability to multi-task and work in a fast-paced environment with little supervision.
• Working knowledge of basic services provided by skilled nursing facilities.
• Excellent customer service skills with commitment to compassionate care.
• You are a team player dedicated to supporting colleagues.

• Assist with admissions, discharges, and MDS testing as needed.

The Social Services Designee assists in providing social services to residents and their families to assist them in dealing with the impact of illness and extended care placement. This position participates in resident care management and ensures that needs are met through Social Service programming and appropriate referrals to outside agencies and community resources.

How You Will Help People Every Day

• Assist in meeting the psychosocial needs of each resident and their families in coping with the effects of illness, disability, treatment, or stay in the facility.
• Coordinate services with other disciplines within the facility to enable the resident and families to promote their own well-being.
• Maintain a working knowledge of residents’ rights.
• Perform timely documentation of Social Service interventions and assessments in resident charts.
• Participate in resident care management, including resident care plan conferences and developing multi-disciplinary plans of care.
• Inform residents and/or responsible parties of change of roommate(s) and room changes.
• Assist with admissions, discharges, and MDS testing.

What you will need to become a part of our EPIC team:

• Bachelor’s Degree in related human services field, including but not limited to Social Work, Sociology, Special Education, Rehabilitation Counseling, and/or Psychology; LSW preferred but not required
• 1 years of social work experience in a health care setting working directly with individuals;
• Long-Term care experience is preferred.
• Excellent computer skills; experience with electronic charting a plus.
• Must be able to multi-task and work in a fast-paced environment with little supervision.
• A working knowledge of the basic services provided by skilled nursing and related facilities.
