Job summary

Marshall Rehabilitation & Nursing is hiring a Social Services Coordinator to support residents and families and assist with admissions and discharge planning.

In this full-time role, you will coordinate care planning with an interdisciplinary team, address resident and family concerns, and ensure smooth documentation and coordination with outside services. Join our team to make a meaningful difference in residents' lives.

Qualifications

• Social services experience in skilled nursing or long-term care preferred.
• Knowledge of admissions and discharge planning.
• Strong communication and organizational skills.
• Ability to work effectively with residents, families, and an interdisciplinary team.
• Bachelor's degree in Social Work or related field preferred.
