Job summary

Avamere Rehab of Newport seeks a Social Services Coordinator to join our compassionate interdisciplinary team. This full-time, day-shift role focuses on assessing residents' psychosocial needs, coordinating care plans, and ensuring safe discharges while advocating for residents' rights.

You will partner with families, healthcare providers, and community services, manage Medicaid applications, and support post-discharge placement.

Qualifications

• Bachelor's degree preferred in public health administration, social work, or related field.
• Experience with electronic medical records and computer documentation systems.
• At least 1 year of experience in a skilled nursing facility or similar healthcare setting.

Responsibilities

• Identify residents' emotional, mental, and psychosocial needs and maintain current care plans.
• Coordinate discharge plans with the interdisciplinary team to ensure safe transitions.
• Communicate with residents, families, providers, and programs on a regular basis.
• Advocate for resident rights and ensure best interests are served.
• Coordinate post-discharge supports with home health, hospice, and durable medical services.
• Arrange transportation to/from ancillary appointments.
• Lead regular care conferences with residents and families.
• Monitor mood/behaviors and assist with mental health supports.
• Work with insurers to secure payment and handle non-coverage notices.
• Assist with Medicaid applications and spend downs.
• Assist with placement in outside facilities after discharge.
