Springfield Suites Rehab & Nursing Center is seeking a full-time Social Services Coordinator to join our skilled nursing team. You will oversee social services, admissions support, and discharge planning while serving as a key resource for residents and their families throughout their stay.

The role involves coordinating care planning, documenting assessments, and ensuring smooth transitions to home care or other services as needed.

Qualifications

• Experience in social work, admissions, and/or discharge planning in a nursing home or long-term care setting.
• Knowledge of the Medicaid process.
• Degree in Social Services, Human Services, Social Work, or a related field preferred.
• Strong communication, organization, and problem-solving skills.

Responsibilities

• Complete social services assessments, including BIMS, PHQ-9, and trauma screenings.
• Review admission documentation for appropriateness of placement.
• Coordinate discharge planning and aftercare services.
• Arrange home care, medical equipment, follow-up appointments, and community resources.
• Communicate with residents and families regarding concerns and care needs.
• Coordinate care plan meetings.
• Maintain advanced directives and resident records.
• Document assessments and services in PointClickCare.
