Overview

The Social Services Coordinator will work to enhance the quality of life of senior housing residents. This is accomplished by determining service needs, providing access to the entire service system, ensuring effective and coordinated delivery of service to each individual client, and facilitating senior participation in programs that enhance their physical, social, and mental well-being. Service coordination is provided to residents of senior housing owned, operated, and/or managed by NewCourtland.

Minimum Requirements

Education: A bachelor’s degree, preferably in social work or a related field.

Experience: One year in the aging or human service system preferred; a combination of education and experience demonstrating knowledge of the human service system and working with older adults and persons with disabilities is essential.

Responsibilities
  • Provide assistance and advocacy related to social and supportive services to residents; inform about programs such as Medicare, Medicaid, and entitlements.
  • Conduct outreach to new residents to welcome them, inform about available services, and facilitate a smooth transition.
  • Perform comprehensive assessments and periodic reassessments to develop care management plans.
  • Offer counseling and assistance to residents and families for problem resolution.
  • Maintain connections with community resources like the Area Agency on Aging to stay current on services.
  • Determine interventions for crisis situations, conflicts, or issues impacting residents, including Protective Services, abuse, mental health deterioration, and family conflicts.
  • Identify gaps in services and develop innovative solutions.
  • Act as a liaison between residents and management, balancing advocacy and operational needs.
  • Covid vaccination is required unless an approved medical or religious exemption is obtained.
