Responsibilities:

The Service Coordinator’s responsibilities are:

  • Development of the person-centered service plan that addresses participant-identified areas of need, goals, intervention, and progress of goals
  • Coordinate services for participants in relation to their identified needs
  • Monitor authorized services through phone calls, face-to-face visits, person-centered planning team meetings, and contact with providers

The Service Coordinator position can be performed remotely and will require travel within Bucks, Chester, Delaware, Montgomery, and Philadelphia counties.

Education/Experience:

Position Requirements:

  • Must have a Bachelor's degree in social work, human services, psychology, or other related fields with practicum experience, or in lieu of a Bachelor's degree, have at least three (3) or more years of experience in a social service or healthcare-related setting.
