Job summary

Dignity Senior Living in Arlington, VA seeks a Social Services Coordinator to lead assessment and coordination of social services for residents in our Skilled Nursing Centers. You will promote Resident Centered Care by addressing psychosocial needs, supporting families, and ensuring compliance with federal, state, and local regulations, upholding Sunrise’s Mission.

This role oversees admissions, care planning, and collaboration with leadership on budget and staff engagement to deliver

Qualifications

• Assess residents’ psychosocial needs and document care plans per regulatory requirements
• Lead and participate in family and care plan conferences
• Provide counseling and one-on-one support to residents and families
• Coordinate admissions, discharges and room transfers
• Complete and support MDS and care planning processes
• Act as liaison with community agencies and external resources
• Support Resident Council and coordinate transportation as needed
• Ensure compliance with social services regulations and Sunrise standards
• Manage departmental operations, staffing, scheduling and engagement initiatives
• Partner with leadership on budget management and financial oversight
