Job summary

Southern HealthCare Management is seeking a Social Services Assistant for our nursing center in Florida. You will support admissions, psychosocial assessments, care plans, and discharge planning, ensuring a comfortable experience while completing treatment goals.

Enjoy a Monday–Friday dayshift with benefits, and work closely with the Social Services Manager to help residents and families.

Qualifications

• High school diploma or equivalent.
• Bachelor's degree in Social Work or in a human services field.
• One year supervised social work experience in a health care setting.
• One year of health care education/marketing/insurance experience preferred.
• Familiar with medical terminology and proficient with computers.

Responsibilities

• Welcome and admit new residents and patients.
• Complete psychosocial assessments and care plans.
• Develop discharge plans and ensure a positive experience during treatment.
• Support the social service function alongside the Social Services Manager.

Education

Bachelor's degree in Social Work

Bachelor's degree in a human services field (sociology, gerontology, etc.)

As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday – Friday, dayshift work schedule.

Qualifications

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.
