Job summary

Mountain View Post Acute is seeking a Social Services Assistant to aid in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs under the direction of the Social Services Director. This role supports residents and families, coordinating services and community resources within a post-acute care setting.

• Prior health-care experience preferred.
• Experience in a post acute or skilled nursing facility preferred.
• Associate or Bachelor’s Degree preferred but not required.
