Job summary

Mountain View Post Acute in Mountain View, CA seeks a Social Services Assistant to contribute to social service programs under the Social Services Director. This full-time role runs Tuesday through Saturday, with a focus on resident support and resource coordination.

The successful candidate may have healthcare experience; high school diploma required, associate or bachelor’s degree preferred. Compensation is $20–$22 per hour, with full-time benefits and growth opportunities.

Qualifications

• Prior health-care experience preferred.
• Experience in a post acute or skilled nursing facility preferred.
• Must possess, as a minimum, a high school diploma or its equivalent.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs under the direction of the Social Services Director.
• Support residents and families by coordinating resources and referrals as needed.
