Job summary

The Nashville-based nursing home is seeking a Social Worker to join its care team, focusing on residents’ social, emotional, and discharge planning needs.

Candidates should have a BSW/MSW or related degree and at least 3 years of Social Work experience, with flexible scheduling and PCC experience preferred.

The role requires collaboration with interdisciplinary teams, finance for resident education and documentation, and planning for admission, re-entry, or transfer.

Qualifications

• At least 3 years experience in Social Work.
• Ability to work flexible schedule.
• PCC experience preferred.

Responsibilities

• Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needs.

• Developing and maintaining individualized care plans.
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility.
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident.
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay.
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community.
