Job summary

CareRite Centers, LLC is seeking a Social Worker for a Nashville nursing home to support residents’ social and emotional needs and coordinate individualized care plans.

The role includes participating in admission planning, interdisciplinary meetings, and discharge planning to ensure safe transitions back to the community.

Qualifications

• BSW or MSW preferred, or Bachelors degree in the related field
• At least 3 years experience in the Social Work field required
• Ability to work flexible schedule
• PCC experience preferred

• Maintaining and/or improving the resident's ability to control everyday physical needs and mental/psychosocial needs
