Job summary

Carmel Hills Wellness & Rehabilitation is seeking a compassionate Social Services Designee to support residents and families in a skilled nursing setting. You will conduct psychosocial assessments, contribute to care plans, and advocate for resident rights with a person-centered approach.

Qualifications include state certification or licensure, a Bachelor’s in Social Work or related field, and at least 1 year of healthcare social work experience; SNF experience is preferred.

Qualifications

• Bachelor’s degree in Social Work or related Human Services field.
• Certified, licensed, or registered in the state of practice (required).
• Minimum of 1 year of social work experience in a healthcare or geriatric setting.
• SNF experience preferred.

Responsibilities

• Complete MDS-related documentation accurately and in compliance with regulations.

• Assist with survey readiness and regulatory compliance efforts.
