Job summary

Monarch Springs Wellness & Rehabilitation seeks a compassionate Social Services Designee to support residents’ emotional, social, and psychological well‑being. You’ll assess, plan, advocate, and coordinate with the care team to ensure person‑centered care and smooth transitions.

Requirements include state licensing, a Bachelor’s in Social Work or related field, and at least 1 year in healthcare or geriatric social work; SNF experience is preferred.

Qualifications

• State-certified, licensed, or registered in the state of practice.
• Bachelor’s degree in Social Work or related Human Services field.
• At least 1 year of social work experience in a healthcare or geriatric setting.
• Skilled Nursing Facility experience preferred.
• Nursing experience in a SNF is a plus.

Responsibilities

• Support residents through psychosocial assessments and individualized care planning.
• Help residents and families navigate the transition to long‑term and memory care.
• Complete MDS‑related documentation and maintain accurate, compliant records.
• Advocate for resident rights, dignity, and self‑determination.
• Coordinate discharge planning and community resources.
• Collaborate with nursing, administration, and the interdisciplinary team.
• Assist with survey readiness and regulatory compliance.

What We’re Looking For
