Job summary

Windsor House Inc. is hiring a Social Worker for Liberty Health Care Center in Youngstown, OH. The role focuses on organizing and coordinating resident care, with emphasis on initial and ongoing assessments to meet needs.

The ideal candidate will have a degree in social work (LSW preferred) and excellent written and verbal communication skills. Flexible hours are required to support resident care and family communications.

Social Worker Responsibilities:

• Organizing, developing and coordinating aspects of resident care.
• Initial and ongoing assessments required to meet resident needs.
• Liaison between the resident, family & staff regarding concerns, which may arise.

Qualifications

• At least one year experience in Social Service work in a long term care setting preferred but not necessary
• Must be willing to work flexible hours.
• Must have excellent communication skills, written and verbal.
• LSW or a degree in social work required, a degree in similar field with experience on long term care will be considered.
