Job summary

You will coordinate care plans, supervise social work staff, and ensure compliant documentation using PointClickCare. Requirements include a BSW or related field and a state credential; two years in long-term care; strong communication.

Qualifications

• Bachelor’s degree in Social Work or related field and state credential.
• At least two years of experience in long-term care.
• Excellent verbal and written communication skills.
• Experience with MDS, CAA and CP, BIMS and PHQ9 preferred.
• Ability to multi-task and work in a team environment.
• Compassion for the elderly and families.
• Investigation skills related to resident concerns and complaints.
• Electronic charting proficiency; Windows suite knowledge.
