As a Social Service Director , you will be responsible for overseeing the Social Services department, which provides emotional support, counseling, and linkage to community resources for our patients. Responsibilities

• Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team

• Provide emotional support and counseling to patients and families, addressing emotional, social, and financial concerns

• Connect patients with community resources, such as home healthcare, adult day care, and long-term care facilities

• Coordinate discharge planning and ensure a smooth transition for patients returning home

• Supervise and mentor Social Workers and other support staff to ensure high-quality care and support

• Participate in quality improvement initiatives to improve patient outcomes and satisfaction

• Maintain accurate and up-to-date records of patient care and services provided
