Canyon Creek Post-Acute in Castro Valley, CA is hiring a Social Service Staff member to support residents’ physical, mental and psychosocial needs under the Social Service Director.   • Familiarity with Medicare processes and regulations is a plus.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Strong interpersonal skills with the ability to build rapport with diverse populations.
  • Excellent organizational skills and attention to detail for maintaining records.

Responsibilities

  • Assist with resident admissions and personal arrangements: admission processes, facility tours, inquiries, and coordination of personal care needs (hair care, transportation, adaptive equipment, clothing).
  • Maintain communication and documentation: contact families/physicians, invite to care plan meetings, document interactions and referrals in the log.
  • Support residents and families with forms and applications: guide in completing advance directives, Medi‑Cal applications, voter registration, and other documents.
  • Assist in resolving issues: address resident/family grievances, theft, and loss concerns.
  • Participate in programs and meetings: join behavior management plans, quality assurance programs, in‑service trainings, and staff meetings.
  • Ensure compliance and confidentiality: follow policies, laws, regulations; maintain confidentiality and residents’ rights.
  • Demonstrate professionalism and safety: be a role model, punctuality, safety standards, respond to emergencies.
