Job summary

Vasona Creek Healthcare is seeking a compassionate Social Service Assistant to join our team on a part-time basis. You will collaborate with our Social Services team to support patients, residents, and families, ensuring emotional support, access to community resources, and effective discharge planning.

Applicants should have a high school diploma; an Associate’s in Social Work, Psychology, or related field is preferred. Prior healthcare experience and strong EHR skills are desirable.

Qualifications

• Experience in long-term care, skilled nursing, or healthcare setting preferred.

Responsibilities

• Assist residents and families with admission and discharge planning.
• Identify social and emotional needs of residents and coordinate interventions.
• Maintain confidential documentation and electronic social service records.
• Coordinate with community agencies to secure external resources for residents.
• Help residents resolve grievances or concerns to ensure a positive care experience.
• Participate in multidisciplinary care planning meetings.

Qualifications & Skills

• Communication: Strong interpersonal skills with the ability to speak empathetically with residents and families.

• Computer Skills: Proficiency in electronic health record (EHR) systems and basic office software.
