Vasona Creek Healthcare is seeking a compassionate and organized Social Service Assistant to join our team part-time. You will work closely with our Social Services team to support patients, residents, and their families. Qualifications

• High school diploma required; Associate's in Social Work, Psychology, or related field highly preferred.
• Experience: Previous experience in a long-term care, skilled nursing, or healthcare setting is preferred.
• Communication: Strong interpersonal skills with the ability to speak empathetically with residents and families.
• Organization: High attention to detail and ability to manage multiple administrative tasks.
• Computer Skills: Proficiency in electronic health record (EHR) systems and basic office software.

Responsibilities

• Assist residents and families with admission and discharge planning.
• Identify social and emotional needs of residents and coordinate appropriate interventions.
• Maintain accurate, confidential documentation and electronic social service records.
• Coordinate with community agencies to secure external resources for residents.
• Help residents resolve grievances or concerns to ensure an optimal care experience.
• Participate in multidisciplinary care planning meetings.
