Job summary

Waters Edge Health and Rehab in Wisconsin is seeking a Social Service Assistant to join our social services team. You will work closely with residents to support their social and emotional needs and assist with admissions and transfers as part of the care plan.

This full-time role has day hours, collaborates with an interdisciplinary team, and offers growth opportunities in a compassionate, resident-focused facility dedicated to dignity and quality of life.

Qualifications

• Experience in long term care or SNF.
• Experience working with a diverse aging population.
• Social work setting experience is preferred.

Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Waters Edge Health and Rehabilitation Center is seeking a Social Service Assistant to join our social services department. As a Social Service Assistant, you will work closely with our residents to ensure quality care is provided and improve the lives of our residents.

This is a full time position working Monday - Friday day hours.
