Job summary

Burlington Health and Rehabiliation Center is seeking a Social Service Assistant to join our social services department and work closely with residents to ensure quality care and improve their lives.

This is a full-time position with Monday–Friday day hours. Duties include planning admissions, coordinating with interdisciplinary teams, and assisting with discharge planning and necessary documentation.

Qualifications

• Experience in long term care or SNF.
• Experience working with a diverse aging population.
• Social work setting experience is preferred.

Responsibilities

• Maintaining and/or improving residents' ability to control everyday physical needs and mental/psychosocial needs.
• Identifying individual social and emotional needs.
• Participating in the planning of admissions, community re-entry or transfers.
• Participating in interdisciplinary care plan meetings to address social and emotional needs.
• Work with the finance team to ensure residents' documentation is in place.
• Assist with discharge planning and arrange necessary DME or home care.

Burlington Health and Rehabiliation Center is seeking a Social Service Assistant to join our social services department. As a Social Service Assistant, you will work closely with our residents to ensure quality care is provided and improve the lives of our residents.

This is a full time position working Monday - Friday day hours.

Some duties may include:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the communit

Job Qualifications:

• Experience in long term care or SNF
• Experience working with a diverse aging population
• Social work setting experience is preferred
