Job summary

Waters Edge Health and Rehabilitation Center in Kenosha, WI, is seeking a Social Service Assistant to join our social services department. You will work closely with residents to support quality care and improve lives.

This full-time role operates Monday–Friday day hours. You will plan admissions, coordinate discharge, and collaborate with the interdisciplinary care team to address social and emotional needs.

Qualifications

• Experience in long term care or SNF.
• Experience working with a diverse aging population.
• Social work setting experience is preferred.

Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
