Job summary

CareAparent is seeking an in-home Registered Nurse (SOC/ROC RN) to assess, plan, and supervise care for seniors in Minnesota. You’ll administer care plans, coordinate with therapists, and educate patients and families while using our EMR system to document care.

Responsibilities include initiating admissions, supervising LPNs and CNAs, and maintaining compliance with state/federal guidelines. On-call duties evenings/weekends are required, with competitive hourly wages and mileage reimbursement.

Qualifications

• Graduation from an accredited nursing program; BSN preferred.
• Minimum of one year of home care or assisted living experience.
• Valid RN license in Minnesota.
• Valid driver's license with insured vehicle.
• Knowledge of quality improvement and home health standards.
• Understanding impact of acute and chronic illness on patients and families.

Responsibilities

• Conduct initial and ongoing home health OASIS assessments and care planning.
• Develop and monitor plans of care based on physician orders and assessments.
• Coordinate referrals to interdisciplinary team (PT, OT, Speech, Falls Prevention).
• Perform medication set-ups under Clinical Manager guidance.
• Supervise LPN visits and ensure plan of care is followed; report to Case Manager.
• Oversee CNA/PCA/HHA visits and ensure quality of care delivery.
• Document client assessments and visits using CareAparent EMR.
• Work on-call for rotating evenings and weekends; maintain RN license.
