Job summary

CareAparent is seeking an Admissions Registered Nurse (SOC/ROC RN) to serve the West side of the Twin Cities Metro. You will perform in-home OASIS assessments, help prepare care plans, and collaborate with a multidisciplinary team to ensure safe admission of home health episodes.

You’ll educate patients and families, support providers, and follow CMS and Joint Commission guidelines. The role requires Minnesota RN licensure, a valid driver’s license, and a minimum of one year in home care.

Qualifications

• RN license in Minnesota required.
• Minimum 1 year of home care experience.
• Valid driver’s license with insured vehicle.
• BSN preferred (not mandatory).
• Knowledge of quality improvement and home health standards.

Responsibilities

• Conduct initial and ongoing OASIS assessments in home care.
• Develop, monitor, and update plans of care based on assessments.
• Coordinate referrals to interdisciplinary team (PT, OT, Speech, Falls Prevention).
• Perform medication set-ups under Clinical Manager guidance.
• Document client assessments and visits in CareAparent EMR.
• Work on call for rotating evenings and weekends.
