CareAparent is seeking a Start of Care/Resumption of Care Registered Nurse (SOC/ROC RN) to perform in-home assessments and care planning in Minnesota. The role includes supervising HHA and CNA staff, delivering medication set-ups, and educating patients and families.

The RN ensures the home health plan aligns with physician orders and state guidelines. The position requires an active Minnesota RN license, a valid driver’s license, and the ability to work across multiple zones with potential

Qualifications

• Graduation from an accredited nursing program; BSN preferred.
• Minimum of 1 year of home care or assisted living experience.
• Valid Registered Nurse license in Minnesota.

Responsabilités

• Conduct initial home health care OASIS assessments (SOC/ROC/Recertifications).
• Develop and monitor care plans based on physician orders and patient needs.
• Supervise LPN visits and coordinate CNA/PCA/HHA on-site care.
• Perform medication set-ups under Clinical Manager guidance.
• Maintain RN license and be on-call for evenings/weekends.
