Comprehensive Rehab Consultants seeks a part-time NP/PA/PM&R clinician in Antioch, TN. Provide PM&R services at a designated SNF with autonomy over daily workflow and collaboration with the interdisciplinary team.

Qualifikationen

• License as NP/APRN/PA in Tennessee
• Ability to work in the U.S.
• Post-acute space experience preferred
• Strong interpersonal and patient-care skills

Aufgaben

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings
• Represent CRC in the facility with clinical leadership and collaboration with the care team
• Collaborate with PCP teams on conditions common in SNFs (CVA, neurodegenerative conditions, postoperative ortho)
• Work with PT/OT/SLP to improve patient function and quality of life
• Attend multidisciplinary meetings and bridge therapy with internal medicine
