The ideal candidate brings post-acute experience, Tennessee licensure, and a passion for geriatric rehabilitation.

Qualifications

• Post-acute/post-acute space experience preferred.
• Autonomy in clinical workflows and strong patient focus.
• Ability to collaborate with diverse care teams in SNF settings.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Lead and represent CRC in the facility with the care team.
• Collaborate with PCPs to manage common SNF conditions.
• Work with PT/OT/SLP to improve patient function.
• Attend multidisciplinary meetings and contribute to culture-building.
