Job summary

Comprehensive Rehab Consultants is seeking a Part-time PM&R Clinician (NP/PA) to deliver specialized Physiatry services in a designated SNF/post-acute center in Antioch, TN. You will lead clinical care, collaborate with the therapy team, and help manage conditions common in post-acute populations.

The role emphasizes autonomous practice, strong interpersonal relations with facility partners, and opportunities for ongoing education and professional growth within CRC.

Qualifications

• NP/APRN/PA license required.
• Tennessee license required.
• U.S. work eligibility.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute center.
• Represent CRC with clinical leadership at the facility.
• Collaborate with PCP teams for SNF-related conditions.
• Coordinate with PT/OT/SLP to improve patient function.
• Monitor patient progress in therapy and follow patients across visits.
• Address barriers to therapy (pain, spasticity, neurogenic issues).
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
