Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care.

We expect you to provide excellent patient care, collaborate with facility staff, supervise PTAs, and maintain timely documentation while educating patients and families on progress and outcomes.

Formación

• Evaluate and treat patients following an individualized plan of care.
• Provide high-quality patient care in a SNF setting.
• Collaborate with facility staff to ensure excellent patient outcomes.
• Supervise Physical Therapist Assistants and guide therapy staff.
• Complete timely documentation including evaluations, re-evaluations, discharges, and progress notes.
• Educate patients and families about conditions, progress, and expected outcomes.

Responsabilidades

• Provide excellent care for patients in SNF.
• Deliver outstanding customer service to patients and families.
• Collaborate with facility staff to ensure high-quality care.
• Evaluate new patients and develop plans of care with measurable goals.
• Treat patients and educate them on injury, progress and outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate progress and revise plans of care as needed.
• Complete timely and accurate documentation.
