Job summary

Enhance Rehabilitation is seeking a qualified Physical Therapist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care while collaborating with facility staff to ensure the highest quality of patient care.

You will also supervise Physical Therapist Assistants, re-evaluate patient progress, adjust plans of care, and maintain timely documentation to record evaluations, re-evaluations and discharges.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, educating them on injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
