Enhance Rehabilitation is seeking qualified Physical Therapist candidates to join our dynamic SNF team. The Physical Therapist will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure the highest level of patient care.

Responsibilities include supervising Physical Therapist Assistants, re-evaluating progress, and completing timely documentation to support patient outcomes. Excellent patient service is emphasized in this role.

• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Collaborate with facility staff to ensure excellent care for your patients.
• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes.
• Supervising Physical Therapist Assistants.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes.
