Job summary

Medical Director Services PC in Argyle, NY seeks an Attending Physician (MD/DO) to care for skilled nursing facility residents. The role includes admissions, routine and acute visits, and leadership in care planning within a physician-owned SNF-focused group.

Qualifications

• Active NYS medical license (MD or DO) required.
• Current DEA registration in good standing.
• Up-to-date CME requirements.
• Experience in SNF/long-term care settings preferred.
• Ability to work with a culturally and linguistically diverse resident population.

Responsibilities

• Perform admissions, monthly, acute & routine visits for SNF residents on regulatory basis.
• Collaborate with APPs on your units.
• Manage medication decisions with emphasis on reducing polypharmacy.
• Lead care planning, discharge planning, and ongoing staff education.
• Support the facility through regulatory and survey site visits.
• Coordinate with local hospitals to ensure continuity of care for residents.
