Responsibilities

• Perform admissions and monthly, acute & routine visits for SNF residents
• Collaborate with APPs on your units
• Manage medications with emphasis on reducing polypharmacy
• Lead care planning and discharge planning; educate staff
• Support regulatory and survey site visits for the facility
• Coordinate with local hospitals to ensure continuity of care
