Qualifikationen

• Strong documentation and regulatory compliance practices.

Aufgaben

• Perform admissions, monthly, acute and routine visits for SNF residents.
• Collaborate with APPs on unit care.
• Manage medication decisions, including narcotics and psychiatric meds.
• Lead care planning, discharge planning, and staff education.
• Support the facility through regulatory and survey site visits.
• Coordinate with local hospitals for continuity of care.

Ausbildung

Active NYS medical license (MD or DO)

Current DEA registration in good standing

Up to date CME requirements

SNF/long-term care or managed care experience preferred
