Qualifications

• Active NYS medical license (MD/DO).
• Current DEA registration in good standing.
• Up to date CME requirements.
• SNF/long-term care or managed care experience preferred.
• Ability to work with a culturally and linguistically diverse population.

Responsibilities

• Perform admissions, monthly, acute & routine visits for SNF residents.
• Collaborate with APPs on the units.
• Manage medications including narcotics and psychiatric meds to reduce polypharmacy.
• Lead care and discharge planning, plus staff education.
• Support facility through regulatory and survey site visits.
• Coordinate with local hospitals to ensure continuity of care.
